Help Us Get to Know Your Child Better

Starting a new journey in a special education setting can feel like a big step — for both children and their families. At Saint Clare School, we want to make that step as smooth and supportive as possible. To do this, we ask all new families to complete our Orientation Form.

This form gives us important insight into your child’s background, daily needs, and behaviours. It ensures our team is prepared to welcome your child with the right level of care, structure, and attention from the very first day.

Purpose of the Orientation Form

The Orientation Form helps us:

  • Understand your child’s personality, routines, and preferences

  • Plan appropriate classroom placement and therapist support

  • Communicate effectively with your family from day one

  • Promote a smooth adjustment to our school environment

  • Prepare our team with safety and care measures specific to your child

What You’ll Provide

You will be asked to fill in detailed information across the following areas:

  • Personal information, language spoken, family details

  • Communication methods, behaviours, and social habits

  • Medical history, allergies, and existing therapies

  • Eating, toileting, sleeping, and sensory preferences

  • Emergency contacts and consent acknowledgements

This form helps us work as a team — with you — to ensure your child feels safe, understood, and supported right from the start.
